Axon is designed for patients who have been diagnosed with chronic pain, which is defined as pain lasting continuously for 3 months.

This questionnaire takes about 3 minutes to complete.

Pre-screening

Please Acknowledge the Following Conditions:

1. I am OK with wearing a headset that feels like headphones for 30 minutes at a time.
2. I understand that completing 5 sessions per week over the course of 12 weeks gives me the best chance of reducing my pain.
3. I also understand that to maintain my pain relief, I will need to perform additional sessions on an ongoing basis.
4. My hairstyle does not have permanent braids or twists that are styled in dreadlocks, as this may impede the performance of the Axon headset.
5. I do not have any of the following or I have sought medical advice from my GP on:
   • Head injury or concussion within the past 6 months
   • History of seizures (e.g., epilepsy)
   • Diagnosis of a serious neurological disorder
   • Undergone treatment for a serious disease (e.g., cancer) within the last 6 months
   • Using an electronic implanted device (e.g., pacemaker, loop recorder)
